Job Summary

Seeking an Illinois Licensed Family Nurse Practitioner, the full-time remote role will manage virtual wellness visits, conduct health assessments, and provide comprehensive care coordination while delivering high-quality healthcare services to patients in their homes.

Key responsibilities

• Conduct virtual adult and pediatric wellness visits and health assessments
• Collaborate with in-home clinical teams to evaluate patient needs and coordinate care
• Provide comprehensive care management, including preventive care interventions and medication reviews

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in Illinois
• Experience performing wellness visits
• Technologically savvy with proficiency in using mobile devices and HIPAA secure messaging apps
• Ability to work autonomously and demonstrate professionalism and compassion in patient care
