Illinois Licensed Family Preservation Case Manager

Job Summary

To support families involved with DCFS, the full-time Family Preservation Case Manager will assess needs, develop individualized goals, and connect families with appropriate services in a hybrid work environment.

Key responsibilities

• Meet with children and families to assess needs and strengths
• Develop individualized goals focused on safety, stability, and family preservation
• Conduct regular home and community-based visits to monitor and document progress

Required qualifications

• Bachelor's or Master's Degree in a Human Services field (Social Work, Psychology, Counseling, or related field)
• Minimum of two years of related experience for Bachelor's degree holders
• CWEL license preferred
• Valid Driver's License in good standing and active automobile insurance
• Ability to obtain an Illinois Driver's License within thirty days of hire if relocating from out of state
