Job Summary

A company is looking for a Field RN Case Manager in Illinois.

Key Responsibilities

• Manage and coordinate care for patients in a remote setting
• Conduct assessments and develop care plans in collaboration with healthcare teams
• Monitor patient progress and adjust care plans as necessary

Required Qualifications, Training, and Education

• Current unrestricted RN licensure in Illinois
• 3-5 years of clinical experience, preferably with Medicare members
• 2-3 years of experience in case management or home health care coordination
• Associate's degree with a requirement to obtain a bachelor's degree within 3-5 years
• Certified Case Manager certification is preferred
