Job Summary

Supporting value-based care programs remotely, the full-time Illinois Licensed Medical Assistant will conduct chart reviews, identify care gaps, and perform outreach activities to enhance patient outcomes, primarily for Medicare populations.

Key responsibilities

• Conduct chart reviews and identify care and quality gaps
• Perform outreach activities to support gap closure initiatives
• Emphasize preventative care and health maintenance to improve patient outcomes

Required qualifications

• Completion of an accredited Medical Assistant program or equivalent experience
• One year of clinical experience preferred
• Medical Assistant Certification/Registration required or to be obtained within 90 days of hire
• Basic Life Support Certification (BLS) required within 60 days of hire
• Strong understanding of quality measures and population health management
