Job Summary

A company is looking for an MTM Pharmacist for remote work within the USA.

Key Responsibilities

• Engage members telephonically and schedule pharmacist consultation appointments
• Utilize prescription claims data to generate clinical recommendations and assess medication regimens
• Document patient care notes and participate in departmental meetings to enhance clinical programs

Required Qualifications

• Bachelor's Degree in Pharmacy or PharmD
• Current and unrestricted Pharmacist license in the state of residence
• Current certification in diabetes care management
• Strong knowledge of disease state management guidelines and MTM services
• Experience working remotely and independently within a cross-functional team
