Job Summary

A company is looking for a Nurse Care Manager - PAC (RN/PT).

Key Responsibilities

• Manage overall care management of patient caseload and facilitate transitions from Skilled Nursing Facilities to home or community
• Collaborate with hospital discharge planners and SNF staff to address patient needs and monitor progress
• Perform assessments to identify patient needs and develop patient-centric care plans

Required Qualifications

• Bachelor's degree in nursing (BSN) with an active RN license or a Bachelor's/Master's degree in Physical Therapy with an active PT license
• Minimum 3-5 years of experience in telephonic care management and post-acute care
• Active license in good standing in the state of Illinois
• Experience in Utilization Management and/or specialty care, including kidney care
• Intermediate skills with MS Office Suite and ability to work effectively in a remote environment
