Illinois Licensed Nurse Practitioner
Job Summary
To support a growing telehealth practice, the full-time Illinois Licensed Nurse Practitioner will conduct thorough patient assessments, develop personalized treatment plans, and collaborate with healthcare professionals while working remotely.
Key responsibilities
- Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
- Develop personalized treatment plans based on patients' needs and medical conditions
- Collaborate with other healthcare professionals to coordinate patient care
Required qualifications
- At least two years of experience practicing functional medicine and performing functional medicine specialty testing/labs
- NP/APRN with a concentration in Functional Medicine, Hormone Health, Anti-Aging, or Lifestyle Medicine
- Active Illinois APRN license required to start employment
- Providers based in FL, MI, TN, and TX are preferred
- Experience in functional and integrative medicine settings preferred
