Job Summary

To support a growing Population Health team, an Illinois Licensed Nurse Practitioner will provide telephonic and video-based follow-up care for patients post-hospital discharge, working part-time or full-time in a remote capacity while focusing on medication reconciliation, symptom screening, and care coordination.

Key Responsibilities

• Manage telephonic outreach and scheduled visits for patients recently discharged from the hospital
• Conduct medication reconciliation, symptom screening, and order follow-up labs and supplies
• Coordinate care across various settings and collaborate with nursing staff to achieve population health goals

Required Qualifications

• Certified Nurse Practitioner in Adult, Family, or Geriatrics
• Active state licensure in Illinois, with the ability to obtain additional state licenses
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Experience engaging Medicare and Medicaid populations managing complex conditions
• Comfortable in a digital-first environment with experience in virtual care delivery
