Illinois Licensed Nursing Program Manager
Job Summary
A company is looking for a Post-Acute Care and SNFist Program Manager for the West Region.
Key Responsibilities
- Facilitate weekly IDT/Medicare meetings to ensure continuity of patient care in skilled nursing facilities (SNF)
- Conduct data collection and chart reviews to evaluate post-acute patient care and identify areas for improvement
- Serve as a liaison between the hospital and SNFs to support safe patient transitions and address concerns
Required Qualifications
- Licensed Registered Professional Nurse (RN) in Illinois
- College diploma in Nursing
- Experience in nursing, particularly in post-acute care settings
- Ability to develop and monitor action plans for quality improvement
- Experience in clinical education and implementing best practices in SNFs
