A company is looking for an RN - Patient Advisory.

Key Responsibilities

• Assess and triage patient concerns via telephone, video, and electronic methods
• Develop care plans and coordinate care for patients experiencing health concerns
• Provide medication management assistance and document all encounters within the EMR system

Required Qualifications

• Licensed Registered Professional Nurse (RN) in Illinois, with additional state licenses required within 1 year
• Education: Diploma in Nursing
• Experience in nursing is required
• Ability to work in a fast-paced environment and provide patient-centric care
• Mastery in chronic and acute care processes
