Job Summary

Seeking an Illinois Licensed Physical Therapist, the full-time remote position will provide telehealth services, focusing on direct patient care, collaboration on treatment plans, and participation in departmental improvement initiatives.

Key responsibilities:

• Maintains primary responsibility for patient plans of care and collaborates with treating PTs and PTAs
• Demonstrates teamwork and effective communication while providing timely updates and addressing patient concerns
• Participates in departmental processes, committees, and educational efforts to enhance service quality and efficiency

Required qualifications:

• Bachelor's Degree in Physical Therapy
• Licensed Physical Therapist in the State of Illinois
• BLS certification from the American Heart Association upon hire
