Job Summary

As a remote full-time Illinois Licensed Physical Therapy Assistant (Care Navigator), the candidate will provide remote monitoring services to support patients in their home-exercise therapy programs, assist users in therapeutic exercises, and maintain records while improving platform processes.

Key responsibilities

• Provide remote monitoring services to assist patients with their home-exercise therapy program
• Direct and support users in therapeutic exercise and functional training through the technology platform
• Maintain necessary records and provide feedback to clinicians on patient progress

Required qualifications

• Active PTA license in Illinois
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with the ability to navigate multiple platforms effectively
• Experience in behavioral modification or Health Coach Certification is a plus
