Job Summary

As a Care Navigator, the full-time Illinois Licensed Physical Therapy Assistant will provide remote monitoring services, assist patients with therapeutic exercises, and maintain communication with clinicians to support patient recovery and musculoskeletal health.

Key responsibilities

• Provide remote monitoring services for patients in their home-exercise therapy programs
• Direct and assist users in therapeutic exercise and functional training through a technology platform
• Maintain necessary records and provide feedback to clinicians on patient progress

Required qualifications

• Active Illinois PTA license
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with experience in multi-platform environments
• Experience in behavioral modification or Health Coach Certification is a plus
