Job Summary

As a Care Navigator, the full-time Illinois Licensed Physical Therapy Assistant will provide remote monitoring services, assist users in therapeutic exercises, and maintain patient records while supporting musculoskeletal health and fostering patient relationships.

Key responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercise and functional training through the technology platform
• Maintain necessary records and provide ongoing feedback to clinicians on patient progress

Required qualifications

• Active PTA license in Illinois
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy, with the ability to navigate multiple platforms efficiently
• Experience in behavioral modification or Health Coach Certification is a plus
