Job Summary

As a Care Navigator, the full-time Illinois Licensed Physical Therapy Assistant will provide remote monitoring services to support patients in their home-exercise therapy programs, assist users in therapeutic exercises, and maintain necessary records while improving platform functionality.

Key responsibilities

• Provide remote monitoring services to support patients' progress through home-exercise therapy programs
• Direct and assist users in therapeutic exercise and functional training via the technology platform
• Maintain records and provide feedback to clinicians on patient progress outside of the clinic

Required qualifications

• Active PTA license in Illinois
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with experience in multi-platform environments
• Experience in behavioral modification or Health Coach Certification is a plus
