Job Summary

A company is looking for a Remote Licensed Practical Nurse (LPN) with Illinois Licensure.

Key Responsibilities:

• Manage patient census and close gaps in clinical and non-clinical patient care
• Identify and coordinate community resources to benefit patient care
• Provide patient education on managing chronic conditions and perform medication management

Required Qualifications:

• Active Illinois License as a Licensed Practical Nurse (LPN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to adapt quickly to a fast-paced work environment
• Must have a home office or HIPAA-compliant workspace
