Job Summary
To provide high-quality patient care services, the Remote Licensed Practical Nurse (LPN) will monitor patients' health remotely, ensuring their well-being while collaborating with healthcare providers, and can work either part-time or full-time.
Key responsibilities
- Use technology to remotely monitor patients' health and provide necessary interventions based on patient data
- Engage with patients through various communication methods to remind them to record vitals and respond to alerts related to their health
- Collaborate with healthcare providers to develop and implement effective care plans and educate patients on using remote monitoring technology
Required qualifications
- Licensed Practical Nurse (LPN) with a valid nursing license in the state of Illinois
- Experience in a healthcare setting, preferably in remote patient monitoring or telehealth
- Comfortable using technology for communication with patients and healthcare providers
- Ability to work independently and as part of a team
- Strong ethics, critical thinking, and problem-solving skills
