Job Summary
To support medically specialized children involved in the child welfare system, the part-time Illinois Licensed Practical Nurse will conduct assessments, coordinate medications, and provide education and advocacy for families, engaging with clients through monthly telehealth and quarterly in-person visits.
Key responsibilities
- Conduct assessments and coordinate medication management for children and youth
- Educate and advocate for families regarding medical and psychiatric treatments
- Maintain treatment coordination through regular telehealth and in-person visits
Required qualifications
- Current Illinois LPN license (Licensed Practical Nurse)
- Experience providing services to children, youth, and families is preferred
- Valid State Driver's License and automobile insurance
- Ability to obtain an Illinois driver's license within 30 days of hire if relocating
