Job Summary
Working part-time during evenings and weekends, the Illinois Licensed Practical Nurse will conduct assessments, coordinate medications, and educate families while providing ongoing telehealth and in-person support for medically specialized children involved in the child welfare system.
Key responsibilities
- Conduct assessments and coordinate medications for children and youth in the child welfare system
- Educate and advocate for families, referring them to community medical and psychiatric treatments
- Maintain treatment coordination through monthly telehealth visits and quarterly in-person meetings
Required qualifications
- Current Illinois LPN license (Licensed Practical Nurse)
- Preferred experience providing services to children, youth, and families
- Valid State Driver's License and automobile insurance
- Must obtain an Illinois driver's license within thirty (30) days of hire if relocating
