Job Summary
To support patient health and long-term wellness, the full-time Remote Illinois Licensed Registered Nurse will manage telephonic patient assessments, chronic condition management, and medication reviews while collaborating with interdisciplinary care teams.
Key responsibilities
- Conduct telephonic triage and prioritize patient assessments
- Provide education to patients and caregivers regarding chronic conditions and preventative care
- Review abnormal laboratory results and support medication management and prior authorizations
Required qualifications
- Active, unrestricted Registered Nurse (RN) license in Illinois
- High School Diploma or equivalent
- 2+ years of telephonic patient assessment/triage experience
- 2+ years of Electronic Medical Record (EMR) experience
- BLS or CPR certification
