Job Summary

To support chronic care management and telephonic triage, the full-time Illinois Licensed Registered Nurse will assess patients, provide education, and manage care remotely from Monday to Friday, ensuring timely interventions and adherence to protocols.

Key responsibilities

Conduct telephonic patient assessments to prioritize care and reduce unnecessary hospital visits
Execute chronic condition management protocols and provide patient education regarding health maintenance
Review high-priority lab results and ensure timely clinical communications and task completion

Required qualifications

Active, unrestricted Registered Nurse (RN) licensure in Illinois
Two years of experience as a Registered Nurse and with Electronic Medical Records (EMR)
BLS or CPR certification
High School Diploma or equivalent; Associate's Degree in Nursing preferred
Proficiency in data management tools such as Google Docs/Sheets
