Job Summary

Supporting patient assessment and telephonic triage, the part-time OSH Registered Nurse will manage chronic condition care, provide education to patients and caregivers, and conduct medication reviews while working remotely on a contract basis.

Key responsibilities

• Support patient assessment and prioritization for telephonic triage to ensure appropriate care
• Execute protocol-driven chronic condition management and provide comprehensive patient education
• Review and respond to abnormal laboratory results to maintain patient safety and continuity of care

Required qualifications

• Active, Unrestricted Registered Nurse Licensure in Illinois
• BLS or CPR Certification
