Job Summary
Supporting patient health and long-term wellness, the full-time Remote Illinois Licensed Registered Nurse will conduct telephonic triage, manage chronic conditions, and provide patient education while working remotely in a temporary position.
Key responsibilities
- Conduct telephonic patient assessments and prioritize care based on clinical protocols
- Provide education to patients and caregivers regarding chronic conditions and preventative care
- Review abnormal laboratory results and manage medication reviews to ensure patient safety and adherence
Required qualifications
- Active, unrestricted Illinois Registered Nurse (RN) license
- BLS or CPR certification
- Minimum of 2 years of telephonic patient assessment/triage experience
- Experience with Electronic Medical Records (EMR)
- High School Diploma or equivalent
