Job Summary

A company is looking for an Illinois Licensed Registered Nurse Care Coach for a remote part-time role.

Key Responsibilities:

• Utilize care management software to call and follow up with Medicare patients with chronic conditions monthly
• Build rapport with patients to coach them on health improvements through SMART goals and self-management education
• Implement and update the Plan of Care, including medication management and connecting patients with community resources

Required Qualifications:

• Current, unrestricted Illinois RN license
• Proficiency with EHRs and web-based applications
• 3 or more years of experience as a Registered Nurse
• Self-directed with the ability to work independently while meeting performance metrics
• Good with technology and eager to learn new software
