Job Summary

A company is looking for a RN Care Navigator to manage case management specific to kidney health management.

Key Responsibilities

• Handle in and outbound calls to deliver service to members
• Educate members on kidney health and coordinate care with healthcare personnel
• Maintain member-centric care plans and assist with ongoing case management

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
