Job Summary

A company is looking for an RN Care Navigator to manage case management specific to kidney health management.

Key Responsibilities

• Handle in and outbound calls, providing exceptional service to members
• Educate members on kidney health, dialysis, and transplant options while engaging them in the Kidney Health Program
• Coordinate care among patients, their support networks, and healthcare providers to resolve issues and assist in achieving health goals

Required Qualifications

• Active, unrestricted RN license in Illinois
• Bachelor's degree required
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
