Job Summary

To support kidney health management, the 100% remote Illinois Licensed RN Care Navigator will manage a caseload of patients, facilitating high-quality care coordination and case management while collaborating with healthcare teams to optimize patient outcomes.

Key responsibilities

• Handle inbound and outbound calls, providing exceptional service to members
• Educate members on kidney health, dialysis, and transplant options while engaging them in HealthMap's Kidney Health Program
• Collaborate with interdisciplinary care teams to coordinate care and assist patients in meeting their individualized health goals

Required qualifications

• Active, unrestricted RN license in Illinois
• Bachelor's degree required
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
