Illinois Licensed RN Care Navigator

Job Summary

To support kidney health management, the remote Illinois Licensed RN Care Navigator will manage a caseload of members, conducting health screenings, creating care plans, and collaborating with healthcare teams to ensure positive patient outcomes.

Key responsibilities

• Handle inbound and outbound calls, providing exceptional service to members
• Educate members on kidney health, dialysis, and transplant options while engaging them in the Kidney Health Program
• Coordinate care as a liaison among patients, their support networks, and healthcare providers to resolve barriers and meet individualized goals

Required qualifications

• Active, unrestricted RN license in Illinois and a compact state
• Bachelor's degree or 6+ years of RN experience, including 3+ years in case management
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
