Job Summary

A company is looking for a Telephonic Case Manager.

Key Responsibilities

• Utilize a collaborative process for assessment, planning, facilitation, and advocacy to meet health needs
• Facilitate appropriate healthcare outcomes by assisting members with appointment scheduling and accessing benefits
• Employ critical thinking and judgment to inform the case management process

Required Qualifications

• Active RN license in Illinois
• Minimum 3-5 years of clinical practical experience
• Preferred 2-3 years of experience in Care Management, discharge planning, or home health care coordination
• Ability to work independently and confidently from home
• Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint
