Job Summary

A company is looking for a Remote RN Case Manager.

Key Responsibilities

• Conduct comprehensive assessments and develop individualized care plans
• Coordinate care across various providers and services to ensure continuity and quality
• Engage with patients and families to provide education and support for health autonomy

Required Qualifications

• Active, unrestricted IL state Registered Nurse (RN) license in good standing
• Minimum of 3-5 years of clinical nursing experience, including care management
• Experience working collaboratively within interdisciplinary teams
• Proficiency with EMRs and comfort learning multiple documentation platforms
• Ability to work independently in a remote environment while maintaining strong team engagement
