Job Summary

To support patients in navigating complex health journeys, the full-time Illinois Licensed RN Case Manager will work remotely to create personalized care plans, coordinate care across providers, and educate members on their health options.

Key responsibilities

• Build meaningful relationships with patients and families to understand their goals and challenges
• Create and implement personalized care plans addressing medical, behavioral, and social needs
• Coordinate care across various providers and settings to ensure safety, quality, and continuity

Required qualifications

• Active RN license with Illinois licensure required
• Strong background in care management and chronic condition management
• Minimum of five years of clinical experience in diverse healthcare settings
• Skilled in motivational interviewing and patient advocacy
• Comfortable using multiple EMR systems and technology in a remote work environment
