Job Summary

Working remotely in a full-time capacity, the Illinois Licensed RN Case Manager will manage care for patients with complex health journeys, focusing on personalized care plans and coordination across providers to enhance wellness and autonomy.

Key responsibilities

• Build meaningful relationships with patients and families to understand their goals and barriers
• Create and implement personalized care plans addressing medical, behavioral, and social needs
• Coordinate care across various providers and settings to ensure continuity and quality

Required qualifications

• Active RN license with Illinois licensure required
• Strong background in care management and complex condition management
• Minimum of five years of clinical experience across diverse healthcare settings
• Experience in motivational interviewing and patient advocacy
• Comfort with technology and multiple EMR systems
