Job Summary

As a full-time remote opportunity, the Illinois Licensed RN Case Manager will manage complex health journeys for patients and families, focusing on personalized care plans that address medical, behavioral, and social needs.

Key responsibilities

• Building meaningful relationships with patients and families to understand their goals and barriers
• Creating personalized care plans that address social determinants and emotional well-being
• Coordinating care across providers and settings to ensure safety, quality, and continuity

Required qualifications

• Active Illinois RN license
• Strong background in care management and chronic condition management
• Minimum of five years of clinical experience across diverse healthcare settings
• Skilled in motivational interviewing and patient advocacy
• Comfort with technology and multiple EMR systems
