Job Summary

A company is looking for an RN Complex Care Manager.

Key Responsibilities

• Engage designated patient populations for complex care and chronic disease management
• Develop and coordinate comprehensive care plans with patients and the care team
• Evaluate and coordinate healthcare services across the continuum of care

Required Qualifications

• Bachelor's Degree in Nursing must be obtained within 3 years of hire
• 3 years of experience caring for patients
• Care management experience with adult and/or pediatric patients with chronic illnesses
• Broad knowledge of medical and community resources
• Current state RN license or license eligible at time of hire
