Job Summary
Facilitating patient transfers into and out of Carle Hospital, the full-time Illinois Licensed RN Specialist will manage communication between referring and receiving physicians, assess patient needs, and ensure timely transportation while working remotely during day shifts.
Key responsibilities
- Expedite consultations and coordinate acceptance, admission, and transfer processes between referring and accepting providers
- Utilize established protocols to ensure smooth transitions and maintain high levels of patient safety and satisfaction
- Document all transfer-related information accurately and provide follow-up communication to ensure continuity of care
Required qualifications
- Licensed Registered Professional Nurse (RN) in Illinois
- Basic Life Support (BLS) certification within 30 days of hire
- College diploma in Nursing
- Minimum of 1 year of experience with Electronic Medical Records (EMR)
