Job Summary

A company is looking for a Telehealth Nurse Practitioner.

Key Responsibilities

Conduct comprehensive health assessments via telehealth
Document visits using ICD-10 and CPT II codes and close HEDIS care gaps
Educate members on next steps and coordinate follow-up if necessary

Required Qualifications

Active Illinois Nurse Practitioner license
APRN or Certified Nurse Practitioner certification
Registered with Medicare and/or Medicaid
Experience in Family Medicine or Internal Medicine
Familiarity with HEDIS and risk adjustment workflows
