Job Summary

A company is looking for a Clerical Territory Patient Admissions Coordinator.

Key Responsibilities

Collaborates with referral sources, sales teams, and clients to collect information for processing quality referrals
Documents referral-related information and actions in the medical record accurately and timely
Ensures timely attention to referrals from various platforms and follows up with clients or caregivers for additional information

Required Qualifications

High School diploma or GED
Minimum of one year experience in home care, hospice, or personal care services
Prior experience with Electronic Medical Records (EMR) preferred
Experience dealing with various healthcare payors, such as Medicare and Medicaid, preferred
