Inbound Care Navigator

Job Summary

Seeking an Inbound Care Navigator, the full-time remote position will provide telephonic and back-office support to clinicians, coordinating care delivery for patients with chronic illnesses through scheduling, referrals, and resource identification.

Key responsibilities:

• Conduct telephonic conversations with patients and families, including triaging calls and making proactive outreach
• Identify community resources and assist patients in accessing them
• Perform various clinical and administrative tasks, including scheduling patient visits and participating in interdisciplinary team meetings

Required qualifications:

• Associates degree or higher education required
• Preferred education/certification as a BSW, CNA, LPN, or MA
• Prior healthcare experience, ideally with chronically ill patients, required
• Experience in patient engagement is highly preferred
• Strong customer-service orientation and exceptional phone etiquette
