Job Summary

To enhance patient care coordination, the full-time RN in Patient Clinical Services will provide telephonic and electronic support to patients in Indiana and Illinois, assisting with triage, scheduling, and health education while ensuring accurate documentation and collaboration with primary care teams.

Key responsibilities

• Assist patients via phone and electronic platforms to assess care needs and provide health information
• Coordinate with primary care physicians and nursing staff to address patient concerns and ensure timely care
• Document patient interactions and maintain electronic medical records, ensuring accuracy and completeness

Required qualifications

• Licensed Registered Professional Nurse (RN) in Indiana and Illinois
• College diploma in Nursing
• Ability to assess and triage patient care concerns based on clinical protocols
• Experience in patient care coordination and electronic medical record management
• Commitment to customer service excellence and professional conduct
