Job Summary

A company is looking for an Admission & Discharge Registered Nurse.

Key Responsibilities

• Conduct initial evaluations to determine patient needs and initiate care plans
• Complete discharge visits and ensure regulatory compliance with care plans
• Coordinate care with other agencies and promote client recruitment

Required Qualifications

• State of Indiana or multistate RN license
• Graduate of an accredited nursing school; BSN preferred
• Home care experience preferred
• Experience with medically fragile clients
• Ability to exercise initiative and independent judgment
