Indiana Licensed Care Coordinator

Job Summary

A company is looking for a Community Well Care Coordinator to manage care for older adult members with complex medical and behavioral health needs.

Key Responsibilities

• Assist with care management for high medical risk members with comorbid behavioral health needs
• Conduct comprehensive needs assessments and develop individualized care plans
• Collaborate with internal teams, providers, and community resources to implement care plans

Required Qualifications

• Bachelor's degree in social work, psychology, counseling, gerontology, or health & human services, or equivalent experience
• 2+ years of experience in long-term support services or working with older adults
• 1+ years of experience with MS Office applications
• Access to a designated quiet workspace at home and high-speed internet
• Must reside in the state of Indiana
