Indiana Licensed Care Manager

Job Summary

A company is looking for a Care Manager to collaborate with an inter-disciplinary care team to improve member care and outcomes.

Key Responsibilities

• Engage members and their support systems through assessments and care planning
• Facilitate communication and coordination among care team members and community resources
• Monitor member progress and satisfaction while managing care plan goals and barriers

Required Qualifications, Training, and Education

• Nursing degree from an accredited program or Bachelor's degree in a healthcare field, or equivalent experience
• Preferred advanced degree associated with clinical licensure
• Minimum of three years of experience in nursing, social work, or related healthcare fields
• Preferred experience in Medicaid and/or Medicare managed care
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor is required
