Job Summary
A company is looking for a Care Manager Registered Nurse.
Key Responsibilities
- Develop personalized treatment plans and support a multidisciplinary team approach for health promotion
- Assess members' needs and document progress in the electronic medical record while collaborating with clinical teams
- Facilitate Care Team meetings and provide education to members on self-care and chronic condition management
Required Qualifications
- Currently licensed Registered Nurse in the state of practice
- Current certification in Basic Life Support for healthcare providers
- Current Wellness Coach Certification or equivalent, to be obtained within 24 months of hire
- Bachelor's degree (BSN) preferred
- At least three years of nursing experience with chronic conditions such as diabetes or heart disease
