Indiana Licensed Care Manager RN

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Develop ongoing care plans and collaborate with providers to ensure members receive necessary services
• Monitor member status and revise care plans as needed to meet goals and address unmet needs

Required Qualifications, Training, and Education

• Must reside in Indiana
• Active RN Indiana or Indiana Compact Licensure required
• Degree from an accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Experience in OB nursing and knowledge of community-based resources preferred
