Indiana Licensed Community Health Physician

Job Summary

A company is looking for a Community Health Physician to provide general supervision for Community Health Workers in local communities.

Key Responsibilities

• Conduct real-time telehealth visits with patients referred by Community Health Workers
• Review Community Health Worker documentation and provide brief monthly notes and billing for services rendered
• Respond to urgent clinical issues and assist patients in finding a primary care provider if needed

Required Qualifications

• MD or DO degree with a valid medical license in Indiana
• Experience in primary care or telehealth services
• Ability to work 5-10 hours per week
• Familiarity with electronic health record (EHR) systems
• Commitment to addressing health inequities in the community
