Indiana Licensed Mom and Baby RN

Job Summary

Collaborating with an inter-disciplinary care team, the full-time Indiana Licensed Mom and Baby RN will engage members through assessments and person-centered planning to improve health outcomes and coordinate care services, primarily via telephonic or electronic communication.

Key responsibilities

• Facilitate strength-based assessments and develop individualized care plans in collaboration with the care team
• Coordinate communication and services among members, providers, and community organizations to address health needs
• Monitor member satisfaction and adjust care interventions based on established guidelines and member preferences

Required qualifications

• Nursing degree from an accredited nursing program
• Minimum of three (3) years of nursing experience
• Preferred three (3) years of Medicaid and/or Medicare managed care experience
• Preferred three (3) years of maternity and/or NICU experience
• Current unrestricted clinical license as a Registered Nurse in Indiana
