Indiana Licensed Mom & Baby Care Manager

Job Summary

A company is looking for a Mom & Baby Care Manager.

Key Responsibilities

• Engage members and their support systems through assessments and care planning
• Facilitate inter-disciplinary care team meetings and coordinate care efforts
• Educate members about treatment options and community resources to support informed decision-making

Required Qualifications

• Nursing degree from an accredited program or Bachelor's degree in a health care field, or equivalent experience
• Advanced degree associated with clinical licensure is preferred
• A minimum of three years of experience in nursing, social work, or related healthcare fields is required
• Three years of Medicaid and/or Medicare managed care experience is preferred
• Current unrestricted clinical license in state of practice as a Registered Nurse, Social Worker, or Professional Clinical Counselor is required
