Indiana Licensed Nurse Practitioner

Job Summary

Providing compassionate, patient-centered care through video visits, the full-time Indiana Licensed Nurse Practitioner or Physician Assistant will focus on remote patient monitoring, hospital discharge follow-up, pre-op anemia management, and E-Visits in a telehealth setting.

Key responsibilities

• Conduct remote patient monitoring and follow-up care via video visits
• Manage pre-operative anemia and coordinate hospital discharge processes
• Utilize fully implemented EMR (Epic) for integrated patient care

Required qualifications

• Licensure as a Nurse Practitioner in Indiana, Illinois, and Kentucky
• Certification by the ANCC/AANP as a Certified Nurse Practitioner or by the NCCPA as a Certified Physician Assistant
• Experience in telehealth or remote patient care preferred
• Ability to work 80 hours every 2 weeks in a full-time capacity
