Job Summary

A company is looking for a Nutritional Support Coordinator to manage patients' nutritional care through remote counseling and care coordination.

Key Responsibilities

• Manage an assigned panel of 300-350 patients with monthly nutritional outreach and support
• Conduct daily phone calls with patients to provide tailored nutritional care plan support
• Collaborate with clinical teams to address nutritional and clinical needs while maintaining documentation compliance

Required Qualifications

• Bachelor's degree in Dietetics or Nutrition
• Minimum 2+ years of clinical nutrition counseling or dietary care coordination experience
• Experience supporting patients with chronic conditions in remote settings
• 2+ years of Electronic Medical Records (EMR) experience
• HIPAA-compliant home office setup with required internet capabilities
