Job Summary

A company is looking for a Social Worker-MSW-Remote.

Key Responsibilities

• Serve as a liaison with faith-based partners and interdisciplinary teams to deliver personalized care
• Assess patients' psychosocial risk factors and provide counseling regarding emotional, social, and financial consequences of illness
• Connect patients and families to necessary community resources and support systems

Required Qualifications

• Master's degree in social work required
• Permanent LSW licensure required; LCSW licensure preferred
• 0-3 years of Medical Social Work experience required
• Knowledge of state and county adult and child protective legal mandates
• Proficiency in Microsoft Excel and Word
